Application Requirements
We hope that our application form is simple and quick to complete. Check the items below to review what information you will need before you make an application.
The application form will require some personal details for the child or young person, the family, and the referring professional. This will include names, contact details, school or childcare locations, previous recreational learning experiences, and details of interactions with other frontline services,
The application form will require details of the chosen activity and provider. This includes the name, location, contact details, and the frequency of activity sessions.
The application form will require consent to store and analyse personal information. Consent must be given by the child or young person, their caregivers, and the referring professional. Consent must also be given for Thriving Through Recreation to use contact details to correspond or discuss an application and/or a grant allocation.
The application form will require a short needs assessment, achievable outcomes, and a rationale for the chosen activity and provider. This is normally completed by the frontline professional who is supporting the application.
The appplication form will require some in-put from the child or young person. If they are aged 8+ there will be space for their own words. The form also requires a short statement from the main care giver(s).
There are three short sections to the application form that must be completed. One relates to the child or young person, one to the main caregivers and/or those with parental responsibility, and another to the professional supporting the application. Ideally, the form is completed together in one sitting. It is designed to take no longer than 15 minutes.
If the form needs to be completed in different sittings by each party, it is possible to submit the form once only partially completed. A link will be automatically generated and emailed. The form can then be accessed and edited further by clicking this link. In this instance please get in touch to inform us if an application form has been submitted but is waiting for further completion.
